How are HLA-DQ2 and blood sugar related?

HLA-DQ2 is a genetic marker strongly associated with celiac disease. Its link to blood sugar is indirect: untreated celiac disease damages the intestinal lining, causing malabsorption that can lead to blood sugar fluctuations (both hypoglycemia and hyperglycemia). Additionally, there is a genetic overlap between celiac disease and type 1 diabetes, as both are autoimmune conditions and certain HLA types (like DQ2 and DQ8) increase risk. Individuals with HLA-DQ2 thus have an elevated risk for celiac disease, and if they also have type 1 diabetes, celiac disease can complicate blood sugar management. Importantly, HLA-DQ2 alone does not predict current blood sugar levels; it is a risk factor that becomes clinically relevant only in the presence of symptoms and antibody tests. The evidence for the association with celiac disease is strong (human-strong), but for direct blood sugar effects it is indirect and mechanistic. A DNA test for HLA-DQ2 is not a substitute for medical diagnosis.
