Transferrin is an iron-transport protein commonly measured as a biomarker of iron status. Some studies suggest a link between elevated transferrin saturation and oxidative stress as well as shortened telomere length – both factors associated with aging. However, the evidence for a direct causal role of transferrin in longevity is weak. Observational studies indicate that both iron deficiency and iron overload (e.g., in hemochromatosis) can be associated with increased mortality. Most consumer tests include transferrin as part of a 'longevity panel,' but interpretation remains speculative. A normal transferrin level alone tells little about individual life expectancy. More important are holistic factors such as diet, exercise, and inflammatory status. Safety note: If iron overload is suspected, a physician should clinically assess ferritin and transferrin saturation – self-tests are insufficient.
