Myths vs facts: FT3 and Stress

A common myth is that stress directly and permanently lowers free triiodothyronine (FT3). In reality, acute or chronic stress can reduce peripheral conversion of T4 to T3 via cortisol-mediated inhibition of deiodinase enzymes (euthyroid sick syndrome). However, this is usually a transient adaptation, not a sign of thyroid disease. An isolated low FT3 without abnormal TSH or fT4 does not indicate hypothyroidism requiring treatment. Many stressed individuals have normal thyroid panels. The evidence is mixed: mechanistic and observational studies exist, but no strong human trials confirm that everyday stress causes clinically significant FT3 deficiency. FT3 testing alone is not validated for stress assessment. If thyroid dysfunction is suspected, a full panel (TSH, fT4, fT3) should be interpreted by a physician. Values also fluctuate diurnally and can be affected by fasting, sleep deprivation, or inflammation.
