A celiac test (typically HLA-DQ2/DQ8 genotyping or tTG-IgA antibodies) is a well-established diagnostic tool. Genetic testing for HLA-DQ2/DQ8 has high sensitivity (>95 %) to rule out celiac disease, as nearly all affected individuals carry these risk alleles. Stress itself does not alter the HLA gene profile – the test result remains stable throughout life. However, chronic stress can impair gut barrier function ('leaky gut') and modulate immune responses, potentially influencing antibody levels (tTG-IgA). Yet no robust studies demonstrate a direct effect of stress on the diagnostic accuracy of celiac testing. The evidence for the genetic test is strong (human association studies), but the link to stress is speculative. A positive HLA result alone does not confirm active celiac disease – additional duodenal biopsy or serology is required. DTC tests offering only HLA are not diagnostic.
