FT4 (free thyroxine) is a key biomarker of thyroid function. A common myth is that a low FT4 level alone causes weight gain, or that raising FT4 (e.g., via supplements) directly leads to weight loss. The fact is: FT4 is only one part of the thyroid profile (TSH, fT3, fT4). Overt hypothyroidism (low FT4, high TSH) can lower basal metabolic rate and promote weight gain, but within the normal range, FT4 has little predictive power for weight loss. Many direct-to-consumer (DTC) tests overemphasize the role of single biomarkers like FT4 in weight regulation. The evidence for a causal link between FT4 and weight loss in euthyroid individuals is weak. Weight loss is multifactorial: calorie balance, exercise, genetics, microbiome, and lifestyle are key.
