The relationship between ferritin (iron storage), iron deficiency, and blood sugar is complex and not fully understood. Ferritin is an acute-phase protein: it rises during inflammation regardless of iron status. Chronic inflammation is linked to insulin resistance and elevated blood glucose. Low ferritin (iron deficiency) may impair insulin sensitivity because iron is needed for glucose utilization in muscle and liver cells. Studies show conflicting results: some find an association between iron deficiency anemia and impaired glucose tolerance, others do not. Additionally, iron deficiency can falsely elevate HbA1c measurements. Importantly, ferritin alone is not a reliable marker for iron deficiency in the presence of inflammation – transferrin saturation is more accurate. The clinical relevance for an individual is limited unless overt iron deficiency or diabetes is present. Evidence: human-moderate (inconsistent). Caveat: ferritin is not a direct blood sugar regulator.
