Can APOE help with fatigue?

APOE variants (ε2, ε3, ε4) are primarily studied for lipid metabolism and Alzheimer's disease risk. A direct link to fatigue is not well supported by evidence. Some observational studies suggest that APOE4 carriers may experience greater fatigue after chemotherapy or in chronic fatigue syndrome, but the evidence is weak and mechanistic. APOE testing is not clinically recommended for fatigue workup. More established causes include iron deficiency, thyroid dysfunction, sleep disorders, or microbiome imbalances. Consumer DNA tests may overclaim relevance. At best, APOE genotyping could be part of a broader risk assessment, but it should not be used alone to explain fatigue.
