HLA-DQ2 is a genetic risk factor for celiac disease, an autoimmune condition that, if untreated, can impair fertility through malabsorption, inflammation, or hormonal imbalances. However, most HLA-DQ2 carriers never develop celiac disease; penetrance is low. Direct evidence linking HLA-DQ2 alone to fertility outcomes in the absence of celiac disease is lacking. Studies indicate that unexplained infertility and recurrent miscarriage are more common in women with active celiac disease, but the genotype alone has no predictive value for fertility. Genetic testing cannot replace clinical evaluation. If celiac disease is suspected, serology (tTG-IgA) and duodenal biopsy are required. The evidence for an HLA-DQ2–fertility link is moderate, based on association studies rather than causal genetics. MyBody-X tests do not provide clinically validated fertility predictions. Caution: a positive result does not indicate current or future fertility problems.
