Whether COMT variants affect fertility is not clearly established. COMT (catechol-O-methyltransferase) metabolizes catechol estrogens, which are estrogen byproducts with potential genotoxic effects. The common rs4680 (Val158Met) polymorphism alters enzyme activity. Some studies suggest links to endometriosis or recurrent pregnancy loss, but findings are inconsistent and effect sizes are small.
