Can SHBG help with blood sugar?

SHBG (sex hormone-binding globulin) is a protein that transports sex hormones like testosterone and estrogen in the blood. Low SHBG levels are epidemiologically linked to insulin resistance, elevated fasting glucose, and higher type 2 diabetes risk. The mechanisms are not fully understood: SHBG may directly influence insulin signaling or simply reflect underlying metabolic dysfunction. However, the evidence is largely observational and not causal. Clinical trials showing that raising SHBG through drugs or lifestyle consistently improves blood sugar are scarce and inconsistent. Therefore, SHBG can be considered a risk marker but not a direct therapeutic target. If SHBG is abnormal, established glycemic markers (fasting glucose, HbA1c) should be assessed. The utility of direct-to-consumer SHBG tests is limited; they do not replace medical diagnosis.
