How are HLA-DQ2 and Allergie related?

HLA-DQ2 is a gene variant complex strongly associated with celiac disease – an autoimmune condition triggered by gluten, not a classic allergy (IgE-mediated). About 25–30% of the population carry HLA-DQ2, yet only a minority develop celiac disease. Testing for HLA-DQ2 has high negative predictive value (ruling out celiac disease) but low positive predictive value. For typical allergies (e.g., pollen, food IgE), HLA-DQ2 plays no established role. Some research explores HLA-DQ2 in non-celiac gluten sensitivity, but evidence is weak and inconsistent. A consumer DNA test for HLA-DQ2 should not be used to diagnose or manage allergies. If celiac disease is suspected, serology (tTG-IgA) and duodenal biopsy remain the gold standard. Evidence: human-strong for celiac disease; unclear for allergies.
