How are SHBG and Wechseljahre related?

SHBG (sex hormone-binding globulin) is a liver-produced protein that binds testosterone and estradiol, regulating their free, active fractions. During menopause (Wechseljahre), estrogen declines, which can initially increase SHBG due to the loss of estrogen's suppressive effect on SHBG synthesis. However, postmenopausally, SHBG often decreases. This shift alters the free androgen index: higher SHBG means less free testosterone, potentially worsening fatigue, low libido, and muscle loss. Conversely, low SHBG is linked to insulin resistance and metabolic syndrome. Measuring SHBG alone is insufficient for menopause assessment; it must be paired with estradiol, FSH, and free testosterone. MyBody-X blood tests can measure SHBG, but interpretation requires clinical context. Caveat: SHBG is affected by thyroid disorders, liver function, obesity, and hormone therapy.
