Risks and limits of Helicobacter Test

Helicobacter pylori testing (breath, stool, or biopsy) is a useful diagnostic tool but has clear limits. A positive result indicates colonization, not necessarily disease – many carriers remain asymptomatic. False positives can occur due to recent antibiotics or proton pump inhibitors; false negatives are possible with low bacterial load or bleeding. Evidence for screening asymptomatic individuals is weak („unclear“), as the benefit of eradication without symptoms or risk factors (e.g., ulcer history, family gastric cancer) is unproven. Moreover, unnecessary antibiotic therapy promotes resistance. Testing should only be performed based on clinical suspicion or guideline recommendations (e.g., Maastricht V/Florence consensus).
