How are TCF7L2 and Müdigkeit related?

The link between TCF7L2 and fatigue (Müdigkeit) is indirect, primarily mediated through type 2 diabetes (T2D). TCF7L2 (transcription factor 7-like 2) is one of the most robustly replicated risk genes for T2D. Variants such as rs7903146 increase T2D risk by about 1.4- to 1.5-fold per risk allele. T2D can cause fatigue via insulin resistance, impaired glucose utilization, chronic inflammation, or associated sleep disturbances. However, most carriers of TCF7L2 risk variants never develop T2D, and even in those with T2D, fatigue is a non-specific symptom. There is no direct, T2D-independent causal relationship between TCF7L2 and fatigue. A genetic test for TCF7L2 alone is not a diagnostic tool for fatigue. Clinical workup (blood glucose, HbA1c, thyroid, iron status) is essential. The evidence for the TCF7L2–T2D association is human-strong (GWAS, meta-analyses), but the link to fatigue is only indirect and mechanistically plausible. In summary: TCF7L2 may contribute to fatigue through increased T2D risk, but it is not a direct fatigue marker.
