MC4R (melanocortin-4 receptor) is a key regulator of energy balance and appetite. Variants in MC4R are strongly associated with obesity, which indirectly affects blood sugar: obesity increases the risk of insulin resistance and type 2 diabetes. Direct effects on blood glucose regulation are modest and not well established. Evidence comes mainly from GWAS and functional studies showing that MC4R mutations lead to hyperphagia and obesity, but the impact on fasting glucose or HbA1c is moderate and context-dependent.
