Elevated tTG-IgA (tissue transglutaminase IgA) suggests celiac disease, an autoimmune enteropathy that impairs nutrient absorption. This can directly affect blood sugar: malabsorption of carbohydrates may cause reactive hypoglycemia or unstable glucose levels. Additionally, celiac disease is associated with type 1 diabetes – about 5–10 % of type 1 diabetics also have celiac disease. Undiagnosed gluten exposure in celiac disease leads to chronic inflammation, which can worsen insulin resistance. Even in non-diabetics, unexplained blood sugar swings, postprandial fatigue, or glucose cravings may point to underlying celiac disease. Important: tTG-IgA alone is not diagnostic – duodenal biopsy remains the gold standard. A gluten-free diet should not be started before confirmation, as it normalizes antibodies and histology. In patients with elevated blood sugar or diabetes, screening for celiac disease is advisable, especially if glycemic control is erratic. Caveat: tTG-IgA can be falsely negative in IgA deficiency; then tTG-IgG should be measured. Evidence for the link is moderate (observational studies, no large RCTs).
