A common myth is that IgE allergy tests (specific or total IgE) can identify the cause of chronic fatigue. In reality, IgE tests measure only immediate-type (Type I) allergies, which typically cause symptoms like sneezing, itching, hives, or anaphylaxis – not fatigue. Fatigue can occur secondarily from untreated allergic rhinitis (e.g., poor sleep), but the IgE test alone is not a diagnostic tool for fatigue. Many fatigue causes (thyroid dysfunction, iron deficiency, sleep apnea, depression) are not detected by IgE. Furthermore, non-IgE-mediated food intolerances (lactose, gluten) are not identified by these tests. Evidence for a direct link between IgE allergies and fatigue is weak and mostly based on observational studies. A positive IgE test does not automatically mean fatigue is allergy-related. Clinically, fatigue should always be evaluated broadly. The test is useful for classic allergy symptoms, not for nonspecific exhaustion.
