Evidence check: APOE in the context of Wechseljahre

Evidence for a specific role of APOE variants (especially APOE4) in menopause is limited. APOE4 is well-studied for Alzheimer's risk and cardiovascular effects, but direct studies on menopausal symptoms or hormonal changes are largely lacking. Some observational studies suggest a possible link between APOE4 and accelerated cognitive decline or adverse lipid changes after menopause, but effect sizes are small and insufficient for clinical recommendations. Hormone replacement therapy or lifestyle interventions should not be based solely on APOE genetics. The utility of genetic testing for menopause guidance is low.
