Is FT4 useful for gut health?

FT4 (free thyroxine) is a thyroid hormone marker, not a direct indicator of gut health. Thyroid function can indirectly affect gut motility, secretion, and microbiome composition – for example, in hypo- or hyperthyroidism – but there is no strong evidence that measuring FT4 alone is useful for assessing or improving gut health. Clinically, FT4 is used to diagnose and monitor thyroid disorders, not gut issues. Some observational studies suggest links between thyroid hormones and intestinal permeability or inflammation, but effect sizes are small and not actionable. For gut health, established markers like fecal calprotectin, zonulin, or microbiome profiling are more relevant. Caveat: Without thyroid-related symptoms, an FT4 test is not indicated for gut health. Evidence: mechanistic/unclear.
