When is MTHFR Basis actually useful?

MTHFR testing (e.g., for C677T or A1298C) is often marketed as a key to personalized nutrition or health, but the scientific evidence for broad utility is limited. These variants are common (30–50% of the population carry at least one) and typically cause only mildly reduced enzyme activity. Clinically relevant usefulness exists mainly when homocysteine levels are elevated – here folate or B12 supplementation can be beneficial. Also in pregnancy (neural tube defect prevention) or with certain drugs (methotrexate), knowing MTHFR status may guide decisions. For the general healthy population, however, the test provides no actionable information. Many direct-to-consumer tests overstate the importance and push expensive supplements without solid evidence. An MTHFR test is only useful when there is a specific medical suspicion – such as hyperhomocysteinemia – and results are discussed with a physician. Without that context, the test mostly creates unnecessary worry.
