CYP2C19 is a cytochrome P450 enzyme primarily involved in metabolizing drugs such as clopidogrel, proton pump inhibitors, and certain antidepressants. A direct causal relationship between CYP2C19 variants and blood glucose levels is not established in the scientific literature. Indirect links have been suggested: for instance, proton pump inhibitors (metabolized by CYP2C19) can alter the gut microbiome, which may influence glucose metabolism. Some GWAS have reported associations between CYP2C19 SNPs and fasting insulin or HbA1c, but effect sizes are very small and replication in large cohorts is lacking. Overall, the evidence is weak to unclear. CYP2C19 genotyping is not clinically relevant for blood sugar management; its primary use is pharmacogenetics.
