What to watch with Transferrin and stress?

Transferrin is an iron-transport protein whose blood levels can be modulated by stress and inflammation. Chronic stress activates the immune system and, via the acute-phase protein hepcidin, alters iron distribution – iron is sequestered in macrophages and liver, and circulating transferrin may decrease. Meanwhile, ferritin often rises as a storage protein. A direct genetic test for transferrin variants (e.g., rs3811647) has no clinical relevance for stress assessment because the effects are small and overshadowed by diet, inflammation, and lifestyle. More useful is measuring transferrin, ferritin, CRP, and iron in blood to detect functional iron deficiency (e.g., due to stress-induced hepcidin elevation). The evidence for a genetic stress-iron axis is currently mechanistic and insufficient for personalized recommendations. Caveat: Without blood values and medical evaluation, DNA tests for transferrin and stress remain of very limited value.
