Cortisol plays a role in labor initiation, but it is not a reliable predictor. In humans, fetal cortisol rises before birth, promoting lung maturation and possibly triggering contractions. However, the evidence is mixed: maternal stress elevates cortisol, yet studies fail to consistently link cortisol levels to labor onset. A single cortisol measurement (blood or saliva) lacks clinical utility for predicting when labor will start. Evidence level: human-moderate (observational studies, no RCTs).
