ApoB (apolipoprotein B) is a direct measure of the number of atherogenic particles (LDL, VLDL) in the blood and is considered a stronger predictor of cardiovascular risk than LDL cholesterol alone. During menopause, estrogen decline leads to increased LDL and ApoB levels, along with a shift toward smaller, denser LDL particles – a particularly atherogenic profile. The evidence for ApoB as a risk marker in menopause is solid (prospective cohorts, meta-analyses). However, randomized trials specifically targeting ApoB in postmenopausal women are lacking. Current guidelines (ESC, AHA) recommend ApoB as an alternative or complementary marker, especially in women with metabolic syndrome or diabetes. An ApoB target < 80 mg/dL (or < 65 mg/dL for high risk) is often suggested. MyBody-X tests can measure ApoB, but interpretation requires clinical context. Caveat: No home test replaces a physician's risk assessment. Evidence: human-moderate (strong association, but no RCTs for menopause-specific cutoffs).
