How are FT4 and menopause related?

The relationship between FT4 (free thyroxine) and menopause is complex and not fully understood. FT4 is a thyroid hormone that regulates metabolism. During menopause, estrogen levels decline, affecting the concentration of thyroxine-binding globulin (TBG). This can cause transient increases or decreases in FT4 without actual thyroid dysfunction. Some studies show mild FT4 changes in perimenopause, while others find no significant association. Additionally, symptoms like hot flashes, fatigue, or weight gain can overlap between menopause and hypothyroidism – differentiation requires repeated lab measurements (TSH, FT4, FT3) and clinical evaluation. A single FT4 value without considering menopausal status is of limited value. The evidence is moderate: observational studies exist, but no strong causal proof. Hormone replacement therapy can alter TBG binding and affect FT4 levels. Important: FT4 alone is insufficient to diagnose thyroid disease; TSH should always be measured concurrently.
